Membership Cancellation

1. Click on the attachment and print it off -OR- include the needed information on your own paper.

2. Simply follow the directions, fill out the sections that are required and mail it to the address provided on the attachment. (We suggest Certified Mail)

Please make sure you include ALL member/s full names, the club they attend, current phone number and the reason for cancellation

The date you sent your request begins the 30 day period for written notice to cancel your membership agreement. You are responsible for any payments during the 30 day period. Memberships with a past due balance or an active Personal Training contract cannot be canceled.

Thank you for providing me with a great workout environment. I regret having to cancel my membership due to moving to a new state.
